To save as PDF: Open this file in your browser, go to File > Print, and choose "Save as PDF" or "Microsoft Print to PDF".
How to Use This Log:
1. Record the date and time of your readings.
2. Enter your blood glucose level. If it's in mg/dL, divide by 18.016 to convert to mmol/L before calculating GKI.
3. Enter your blood ketone level (this should already be in mmol/L).
4. Calculate your GKI: Divide your glucose (in mmol/L) by your ketones (in mmol/L).
5. Use the "Notes" section to record any relevant information (e.g., post-meal, fasting, exercise, how you felt).
